The United Way of Greater Philadelphia & Southern New Jersey announces Aging at Home – A Community Network

10712138895_fe131231c3_bAging at Home – A Community Network is a three-year pilot program designed to help older adults connect with an array of resources supporting their ability to age well at home. Visiting Nurse Association Community Services, Inc. in Abington and Senior Community Services of Lansdowne have been selected to serve as coordinating agencies to pilot the new model beginning in November. Congreso de Latinos Unidos is also designing a plan for implementing the model in the eastern section of North Philadelphia. These well-known local organizations will develop relationships with strategic community providers, stakeholders from the community, policy-makers, and health and social service agencies to coordinate access to local social and health services such as health screenings, chore assistance, transportation and volunteer placement opportunities. If successful, this program will also allow UWGPSNJ to create a sustainable and replicable model of supports that improves health and quality of life for older adults, while reducing their reliance on institutional care and lowering costs for the community overall.
